- Do NOT forget to sign your name
- Use formal name of your organization
- eg. Do NOT use【Doe(株)】for【Doe株式会社】
- Both of the Name of the contracting organization and its representative of the organization is required to write down

- A copy of the statutory report such as the withholding slip for employee salary income for the previous year

- eg. Living in Chiba prefecture you can submit to The Tokyo Immigration office OR Chiba branch office
- For The Tokyo Immigration office you can use the application reservation system
- At the counter you will be asked to write a postcard for noticing you the result

When you received the Notice Postcard(お知らせハガキ) you can take below items with you to pick up your NEW Residence card.

- OLD residence card
- 4,000 yen revenue stamps(収入印紙)
